What is the Purpose of Immigration Physical?
The purpose behind immigration physicals is to ensure that you're mentally and physically fit for residency. To gain residency in the US, you need to have a physical performed by a civil surgeon who has been certified by the US Citizenship and Immigration Services (USCIS). At Houston Concierge Medicine, Dr. Burzynski is a certified Civil Surgeon. He can provide you with the required physical for your residency paperwork.
This is not covered by insurance. Out of pocket only.
Requirements for Immigration Physicals
The full physical exam requires these steps:
- Basic medical immigration physical exam and mental status evaluation
- Blood Test: All adults (from age 18 to 44) are required to have a serologic blood test for Syphilis (RPR).
- Urine Test: All adults (from age 18 to 24) are required to have a Neisseria Gonorrhoeae test.
- Tuberculosis Test: All applicants 2 years of age and older are required to have an IGRA test.
- Proof of vaccinations or blood Titer to prove immunity. Dr. Burzynski will review your vaccination history with you to determine whether you have all the required vaccinations. The cost of additional vaccinations is not included in the price of the basic exam but they are offered through our office for an additional fee. Please bring all vaccination records to your scheduled appointment for the immigration physical.
What Vaccines are Required?
USCIS requires proof of these vaccines:
- COVID-19
- Diphtheria
- The current year’s Influenza
- Hepatitis A & B
- Measles, Mumps, and Rubella (MMR)
- Meningococcal
- Pertussis
- Pneumococcal
- Polio
- Rotavirus
- Tetanus
- Varicella
If you're missing any of these vaccinations, we can provide them at an additional cost.
After the Immigration Medical Exam
After the immigration medical physical examination is completed, the Doctor will certify Form I-693 with the test results of the immigration medical exam and vaccination history in a sealed envelope to present to the INS.
DO NOT OPEN THE SEALED ENVELOPE.
Turn in the envelope with your immigration application. A copy of your blood test results will be provided for you to keep after the results have been discussed with the examining doctor. The examination will only be valid up to a year after the immigration exam date.
How Much Does United States Physical Immigration Cost?
The cost of the Immigration Medical Physical Examination is explained below and includes the following:
- Basic Immigration Medical Physical Examination $250
- IGRA Test $150
- Gonorrhea Test $60
- Syphilis Test $50
- Vaccination status verification
- Copies of your laboratory results
- Certified and Sealed I-693 Form
Additional tests listed below are not included in the price of the exam but are offered for an additional fee. This will add to the cost of the exam. Payment is by cash or credit card. We do not accept major medical insurance for all services required for the I-693 Immigration form.

Our aim is to simplify the process for you as much as possible. If you have more than one person who requires an exam, kindly inform us so that we may book you in for back-to-back appointments. During your exam, all of your medical concerns will be addressed. However, if you have any additional questions or wish to book an appointment, please feel free to give us a call.
